Application Requirements
Just follow the steps below. Be sure to read the section on Selection Criteria for further information.

All applicants need to submit the following:
  • A completed and signed application, including an official academic transcript and a recent, non-returnable photograph of the applicant, labeled with his or her name, address, and telephone number
  • A one-page essay/personal statement about how the applicant is living beyond the boundaries of Crohn's disease. The essay should demonstrate the applicant's academic ambition and personal achievement, and how the scholarship would affect his or her life
  • A minimum of two (maximum of three) letters of recommendation. One letter should come from a teacher or school official and one from a person in the applicant's community (e.g., an employer, coworker, clergy member, or coach)
  • A letter from the applicant's treating physician, confirming a Crohn's disease diagnosis including disease course and severity from their treating physician on the physician's or medical institution's letterhead
